CHANGE IN MEMBERSHIP INFORMATION PROCEDURE

Fill in the Form for change/correction
in Membership Information which is available free of cost at Information
Counter and website of PGSHF. |
2 |
The filled Form should be duly signed by the
member or his/her widow/widower/legal heir(s) in case of death of
member, before its submission to the PGSHF. |
3 |
Copies of supporting documents like CNIC,
Promotion Order, Salary Slip, Retirement Notification, PPO Book etc.
should be attached with the filled Form. |
4 |
Submit the filled Form at Diary Counter of
PGSHF office by hand and get receipt. |
5 |
You may also send filled Form by Post or through
any courier service at PGSHF address i.e. 14-A Upper Mall Scheme,
Lahore. |

CHANGE OF PRIORITY OF STATION
1 |
The member or his/her widow/widower/legal
heir(s) may change priority of station by submitting Form for Change
of Priority of Station or by an Application. |
2 |
Fill in the Form for Change of Priority of
Station(s) which is available free of cost at Information Counter
and website of PGSHF. |
3 |
The Form should be completely filled and signed
by the member or his/her widow/widower/legal heir(s) in case of death
of member, before its submission to the PGSHF. |
4 |
A copy of CNIC of the member or his/her widow/widower/legal
heir(s) in case of death of member, should be attached with the filled
Form/Application. |
5 |
Submit the filled Form/Application at Diary
Counter of PGSHF office by hand and get receipt. |
6 |
You may also submit the filled Form/Application by sending it
through Post or any courier service at PGSHF address i.e. 14-A Upper
Mall Scheme, Lahore.

1) Complete Procedure should be followed.
2 ) Signature on application or change form should match with N.I.C
3 ) Applications are acceptable only by postal mail or by hand.
4 ) No request will be entertained for change if sent electronacilly by
e-mail, fax or through PGSHF web site.
